Enrolling Your Child

In order for your child to be considered for Head Start/Early Head Start, you will need to provide the following information:

A completed application
A copy of a certified birth certificate
A copy of all the income your family receives. For example: 1040 tax return, W-2, two (2) most recent pay check stubs, Public Aid printouts, SSI information, child support or statement of earnings from employer.

The Adobe Acrobat Reader is free software that lets you view and print Adobe Portable Document Format (PDF) files on all major computer platforms.

To view and print PDF files, you may need to download the free Adobe Acrobat Reader to your computer.
